The salary range for this position is $34.42 - $51.63 (Hourly Rate)

Placement within the salary range is dependent on several factors such as relevant work experience and internal equity. For positions represented by a labor union, placement within the salary range is guided by the rules outlined in the collective bargaining agreement.

Description

The Coordinator Mental Health reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

  • The Mental Health Coordinator provides and coordinates all clinical aspects of care for all patients receiving mental health treatment. The coordinator works in collaboration with the unit medical directors and the multidisciplinary team to ensure JCAHO standards and CMS directives for mental health care are met. The coordinator in collaboration with attending psychiatrists, and multidisciplinary team assesses plans and implements care for the mental health treatment programs. By applying evidenced-based decision making, the coordinator serves as a leader, supervisor, and clinical mentor for all members of the BHS care team, with the goal of providing a safe environment and optimal care for all patients. The coordinator will be expected to oversee daily program planning and case assignment, provide clinical supervision for the case therapists and counselors to insure all standards and policies are followed. The coordinator will also provide direct patient care services as needed to assist in meeting patient care needs.  Performs additional managerial duties in the absence of assigned Manager.

Responsibilities:

  • Communicates with family and members of the multidisciplinary team to coordinate care, meeting the specific healthcare needs of the patient.
  • Utilizes appropriate resources for referrals/consultations. Notifies family and care team of scheduled appointments and updates EMR as needed.
  • Assists the Manager in administrative functions, such as organizing and maintaining case therapists schedule and coverage. Assumes duties and responsibilities related to clinical supervision in the absence of Manager. 
  • Identifies criteria for mental health programs and demonstrates knowledge of clinical requirements. Individualizes existing plans of care and develops goals relative to patient needs.
  • Evaluates and documents changes to plans in EMR.  Anticipates impending emergent situations and intervenes effectively, utilizing a wide variety of resources; maintains immediate availability to assist in routine and emergency circumstances as needed.
  • Monitors patient care in all mental health programs to ensure patient safety.
  • Monitors acuity of all programs to ensure patient and staff safety are maintained.
  • Monitors EMR to ensure necessary documentation standards are adhered to by case therapists. Ensures that all new patients are assigned to a case therapist on a daily basis. 
  • Ensures that programming is planned out for each mental health program and that the programming schedule for each is properly staffed. 
  • Assists manager with recruiting, employee engagement initiatives, clinical supervision of staff, and annual appraisals.

Qualifications

Required:

  • Master’s degree in Social Work or Clinical/Counseling Psychology accredited by the Council on Social Work Education (CSWE)
  • Five years of post-graduate school experience in assessment, consultation, and treatment of patients with behavioral health diagnoses in hospital or clinic
  • Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC)
