Description
The Outcomes Manager reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The principal role of the Outcomes Manager in Patient Care Services is to assist the patient care team in achieving the highest quality and best possible outcome for the patient in a cost-effective manner. The Outcomes Manager collaborates with members of the health care team across the continuum. The Outcomes Manager demonstrates a thorough understanding of the demographics for the Palos Emergency Department. Develops approaches to optimize the equity, effectiveness and efficiency of the delivery of resources available to the population both within and beyond the acute care setting.
Responsibilities:
- Outcome Monitoring & Analysis
- Collect, analyze, and interpret clinical outcomes data, including quality metrics, patient safety indicators, and regulatory measures.
- Benchmark performance against national standards and peer institutions.
- Identify gaps in care, emerging trends, and opportunities for improvement.
- Program Development & Implementation
- Design and implement evidence-based strategies to improve clinical outcomes and reduce variability in care delivery.
- Partner with clinical leaders, physicians, and frontline staff to embed best practices into workflows.
- Support the development of performance improvement projects using Lean, Six Sigma, or PDSA methodologies.
- Regulatory & Accreditation Compliance
- Ensure compliance with CMS, Joint Commission, and other accrediting body requirements related to quality and outcomes.
- Prepare reports and data submissions for external agencies and pay-for-performance programs.
- Education & Training
- Educate staff and leaders on outcome measures, performance dashboards, and quality improvement strategies.
- Serve as a resource for evidence-based practice and clinical effectiveness.
- Collaboration & Leadership
- Facilitate interdisciplinary meetings focused on outcome improvement.
- Engage stakeholders in setting priorities and aligning projects with organizational strategy.
- Act as a change agent to promote a culture of quality and patient-centered care.
- Track defect logs (missed bundle elements, near misses) and close the loop on corrective actions within agreed service level agreements
- Serve as the portfolio-based quality subject matter expert (SME), aligning improvements to system initiatives
- Present weekly status to their portfolio-based leadership; contribute data and actions to Quality Committees
- Collaborate with Magnet/Professional Practice to ensure NSI work is reflected in Exemplary Professional Practice narratives and councils.
- Lead assigned projects and interdisciplinary workgroups to advance portfolio-based quality priorities, and perform other duties as required to meet evolving operational and organizational needs
Qualifications
Required:
- Bachelors of Science in Nursing
- RN License
- Two to five years’ experience with the study of patient populations and working with care providers
- CPR – Basic Life Support
- Proficient with MS Office Products, primarily Excel and PowerPoint
Preferred:
- Previous leadership experience
- Masters in nursing strongly preferred
