Description
The Coordinator Patient Carereflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.
The Coordinator Patient Careis a registered professional nurse who in addition to providing direct patient care (detailed in the Registered Nurse BSN job description # 302109) is accountable for the assessment, planning, implementation, and evaluation of a plan of care for designated patients. The Coordinator Patient Care ensures that quality outcomes are achieved and demonstrates exceptional clinical practice. The Coordinator Patient Care reflects the mission, vision, and values of Northwestern Medicine, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.
Responsibilities:
- Serves as a role model for Patients First philosophy.
- Provides direct patient care.
- Provides patient and family education for designated patient populations.
- Assesses, implements, and evaluates the plan of care for designated patients.
- Ensures that quality outcomes are met for designated patients.
- Demonstrates in-depth clinical knowledge, skills, and expertise.
- Ability to integrate theoretical knowledge base and clinical expertise in decision making.
- Assures a cost-effective approach to patient care.
- Participates in development of policies, and other patient care standards.
- Participates in unit/departmental/divisional or hospital committees/task forces as assigned by manager.
Qualifications
Required:
- Registered Nursing license issued by the State of Illinois.
- Bachelor of Science Nursing.
- BLS through American Heart Association.
- Minimum of three years of clinical experience.
Preferred:
- National Certification in area of specialty.
- Master’s Degree.
