The salary range for this position is $27.13 - $33.92 (Hourly Rate)

Placement within the salary range is dependent on several factors such as relevant work experience and internal equity. For positions represented by a labor union, placement within the salary range is guided by the rules outlined in the collective bargaining agreement.

Description

The Licensed Practical Nurse-West Regionreflects the mission, vision, and values of NMHC adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.

A clinically competent, licensed practical nurse who prescribes, delegates, and coordinates nursing care, under the direction of a Physician, or Registered Nurse, to patients through the nursing process of assessment, planning, intervention, implementation, and evaluation.  Effectively interacts with patients, significant others, and other health team members, while maintaining ANA standards of practical nursing practice.

Responsibilities:

  • Performs all aspects of patient care in a manner which optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Utilizes the best available evidence, including research findings to guide practice decisions.
  • Involves patient/family in the formation of the plan of care. Assists in the development, implementation, and updates of an individualized plan of care based on identified patient care needs. Includes nursing, teaching, continuum of care, and discharge planning. 
  • Assesses, analyzes, validates, and integrates data to assess patients' physical psychosocial, rehab, safety, learning, spiritual, cultural, age-specific, special risk, and self-care needs. Identifies patient care needs, problems, and variances. 
  • Adequately assesses and reassesses pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. 
  • Identifies learning needs, abilities, preferences, and readiness; also, identifies support systems, coping mechanisms, and barriers to learning. 
  • Makes referral for follow-up as appropriate.
  • Explains procedures, therapies, and treatments to patient and family, evaluates and documents patient and family response to teaching.
  • Uses appropriate resources to provide education. 
  • Identifies and uses the results of quality improvement activities to initiate changes in nursing practice and in the healthcare delivery system. 
  • Participates in shared governance, committee work, and practice/department projects. Collaborates with the physician and team to review and evaluate the plan of care. Notifies physician of any signs or symptoms that require immediate intervention. 
  • Communicates effectively and works cooperatively with other team members. Has respect for and understanding of other clinical & non-clinical disciplines.
  • Assists in coordinating the care of the patient by including members of the health care team and other departments to plan, deliver, and evaluate patient care. 
  • Delegates appropriately to other care providers to facilitate meeting patient needs in a timely manner.
  • Explains procedures, therapies, and treatments to patient and family, evaluates and documents patient and family response to teaching. Uses appropriate resources to provide education. 
  • Utilizes resources effectively when necessary for patient care issues. 
  • Engages in teamwork as a team player and a team builder.

Competencies

  • Follows the five medication rights and reduces the potential for medication errors.
  • Demonstrates knowledge of medications and their correct administration based on age of the patient and their clinical condition.
  • Demonstrates the ability to state rationale for clinical decisions based on critical thinking.
  • Demonstrates proficiency at coordinating: office flow, rooming patients, triaging calls, ordering tests/referrals, prescription processing, notification of test results.
  • Basic computer knowledge. Ability to communicate effectively in English, both verbally and in writing.
  • Excellent customer service skills.
  • Consistently works within the framework of the Illinois Practice Act for Nursing and ANA Standards of professional nursing practice. Ability to assess and determine situations requiring an interpreter and to use interpreter services effectively.
  • Completion of phlebotomy class or equivalent experience.

Qualifications

Required:

  • Graduate of an accredited LPN program
  • LPN experience within the last five years or, if longer, completion of a refresher course through a recognized educational program.
  • Licensed as an LPN
  • CPR – Basic Life Support
