Description
The Hospital at Home RN Clinical Coordinator reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
This position is an experienced Registered Nurse, who, in addition to providing direct patient care, is accountable for facilitating an interdisciplinary approach to patient care, patient flow, staffing, resource utilization, and problem solving on a given shift.
RESPONSIBILITIES:
Responsible for the 360 clinical, logistic oversight of Hospital at Home operations. This role has daily oversight of the Nurse Hospital at Home Team and is responsible for throughput, capacity management and resource allocation to serve the Hospital at Home patient population to meet the demands across our geographic service areas.
- Facilitates unit operations on a shift basis according to policy, procedure, and regulatory requirements.
- Coordinates unit based staffing assignments on a shift basis to achieve high quality and cost effective care.
- Directs patient care provided by staff nurses and support personnel for a specific shift.
- Champions new initiatives and helps in unit/departmental or hospital wide committees/task forces.
- Communicates directly with patients and significant others about nursing care.
- Implements appropriate action plans to provide safe, quality, and efficient care.
- Assures safety and security of patients, significant others, and staff.
- Provides direct patient care.

- Facilitates effective interdepartmental communication of incidents with appropriate personnel (manager, administrator, medical staff, etc.).
- Promotes philosophy and annual implementation plan objectives of Northwestern Medicine.
- Provides a milieu conducive to communication and cooperation among patients, staff and visitors.
- Identifies and communicates key issues and trends to managers and makes recommendations for improvement on an ongoing basis.
- Provides input to manager on budgetary issues, particularly staffing and cost containment strategies.
- Establishes relationships across departments to optimize patient care.
Education
- Assesses and communicates staff development needs to manager.
- Orients staff to Clinical Coordinator and Charge role.
- Serves as a resource for staff.
- Contributes to unit orientation.
Triage
- Acts as the central contact person in coordinating high level patient placement and patient access for the Hospital at Home system.
- Supports the facilitation of admissions and discharges as needed by the team.
- Serves as key point person for team reporting and clinical operation troubleshooting.
- Anticipates/proactively responds to capacity demands, as necessary.
Case Management
- Patient identification and selection: Focuses on identifying patients who would benefit from case management services.
- Development of the care coordination plan: Develops a plan and prioritizes the patient’s needs, as well as determines the type of services and resources that are available to address the desired outcomes.
Discharge Planning
- Facilitates safe, effective, and efficient discharge plans in collaboration with the patient, the family, caregivers, the attending and/or primary care physician, nursing staff, other appropriate clinical staff, third party payers, and/or community-based care coordinators or other appropriate outside agencies.
Utilization Management
- Provides expert guidance to the medical, nursing, and rehab staff, as well as other Hospital departments regarding Hospital criteria, insurance regulations, care coordination efforts, and discharge planning activities.
- Performs admission and concurrent utilization management using Hospital approved criteria as a guide for determination of appropriate level of care.
- Completes accurate insurance utilization reviews and obtains third party payer approval of services needed for post-hospital care as needed.
Quality /Performance Improvement
- Participates in performance improvement measurements and initiatives including identification of variances/delays.
- Participates in inter-departmental committees, focus groups, task forces, or other special projects as requested.
- Identifies quality of care issues; analyzes, documents, and reports appropriately in accordance with agency and departmental policies and procedures.
Other Duties
- Participates in special projects such as audits, policy and program development, and other such duties as directed.
- Participates and/or carries out other duties as assigned or requested.
Qualifications
Required:
- Baccalaureate degree from an accredited school of nursing or equivalent experience as approved by the Patient Care Director (Minimum two years clinical experience in area of clinical specialty).
- 4+ years nursing experience with progressive supervisory experience
- Current State of Illinois, Registered Professional Nurse license
- Evidence of continued education and professional development.
- Proficiency with triage and managing multiple competing priorities.
- Must have demonstrated clinical competence in area of nursing practice and demonstrated leadership ability.
- Proficiency with Epic
- At point of hire and annually, required to have a valid driver’s license and an annual driving abstract
- American Heart Association BLS
Preferred:
- CCM or ACMA certification
- Prior experience in the following areas:
- general acute care hospital experience
- healthcare reimbursement including utilization review
- quality improvement
- enhanced models of health care delivery
- Previous acute care hospital care coordination/discharge planning
