The salary range for this position is $40.50 - $64.80 (Hourly Rate)

Placement within the salary range is dependent on several factors such as relevant work experience and internal equity. For positions represented by a labor union, placement within the salary range is guided by the rules outlined in the collective bargaining agreement.

Description

The Bed Planner reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

Responsibilities:

  • Determines the most appropriate level of care and ensures a smooth admission experience for patients by coordinating clinical needs with operational processes and insurance benefits.
  • Chart review is used to assess for appropriate level of care.
  • Assigns patients to the appropriate units based on clinical assessment while balancing the resources of the organization.
  • Coordinates with unit charge nurse for bed placement.
  • Collaborates with nursing operations leader for throughput issues.
  • Serves as an access point resource for physicians, staff, and patients.
  • Responds to and assigns all bed request and transfer orders in the electronic medical record.
  • Documents unusual occurrences and patient relations issues; makes appropriate referrals to risk management, infection control and quality departments.
  • Utilizes appropriate downtime procedures for bed placement when applicable.
  • Assists with data collection and reporting for throughput and bed placement.
  • Performs chart audits as requested by manager/director.

  • Collaborates with the admitting physician the most appropriate site of care for services to be rendered and any alternate levels of care available to meet the patients’ needs.
  • Coordinates appropriate bed placement for patients with Patient Care Services, collaborating on location, staffing, etc.
  • Utilizes all available tools and resources to gain better and complete picture of clinical assessment of the patient; notifies physicians and staff of concerns/ variations.
  • Communicates in a non-threatening way that conveys caring and respect for patients, families and co-workers.
  • Communicates a complete and accurate hand-off to the on-coming shift, minimizing miscommunication errors. Complete admission forms and paperwork completely and thoroughly.
  • Assists in easy access to incoming patients by arranging transportation when necessary, coordinating services, and notifying points of entry of patient's impending arrival.
  • Coordinates admission with registration staff to obtain accurate patient demographics as indicated.
  • Refers appropriate cases to patient financial services, risk, peer review, infection control and patient relations.
  • Coordinates patient activity and flow or movement between levels of care. Identifies potential throughput barriers and acts proactively to inform and alert upper management of anticipated delays.
  • Confers with MD to discuss clinical and appropriate level of care when needed.
  • Participates in process improvement activities, identifying opportunities.
  • Demonstrates an interest in personal growth and development beyond the routine expected educational sessions.
  • Embraces new and current co-workers, contributing to their growth and development.
  • Contributes to teamwork and cohesiveness of the department and across departments.
  • Seeks opportunities to network with people in like roles for on-going learning and opportunities to share best practice and process refinement.

Qualifications

Required:

  • Registered Nursing license issued by the State of Illinois.
  • Bachelors of Science Nursing
  • Two years patient care experience in a hospital setting
  • Able to multitask quickly, and process multi factorial stimulation accurately
  • Critical thinking skills, decisive judgment, and the ability to work with minimum supervision

Preferred:

  • Past experience in admissions,  discharge planning, and/or case management
  • Masters of Science Nursing
  • Membership in Professional Nursing Organization
