The salary range for this position is $17.24 - $24.13 (Hourly Rate)

Placement within the salary range is dependent on several factors such as relevant work experience and internal equity. For positions represented by a labor union, placement within the salary range is guided by the rules outlined in the collective bargaining agreement.

Description

The schedule for this new position is 20 hours per week, Monday - Friday between 8a - 5p based on program patient referrals.

The Paramedic for the Mobile Integrated Healthcare program reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

The Paramedic provides patient care within their defined scope of practice that includes patient care, monitoring and observation, in-home patient assessments, clinical and social screenings, clinical interventions, patient education, and participates in telehealth visits from the patient's home while connecting with the physician or designee via telehealth.

RESPONSIBILITIES:

  • Works in collaboration with Advanced Practice Provider (APP) and other members of the care team in providing direct in-home patient care activities including clinical assessment, obtaining vital signs, assisting patients in need of intervention, and participates in telehealth (program provided phone) visits from the patient's home while connecting with the physician or designee via telehealth.
  • Coordinates and maintains equipment and supplies.
  • Documents appropriate assessment, interventions, and other pertinent patient information in a timely manner.
  • Identifies, reports, provides care and documents patient problems in collaboration with the physicians or designee, nurses, and the technical staff.
  • Demonstrates the ability to assist patients’ mobility safely, utilizing mobility devices appropriately.
  • Assists physicians or designee, nurses, and technical staff in providing appropriate patient education including the use of appropriate educational materials.
  • The Paramedic interacts with patients, families, nursing, and a variety of interdisciplinary personnel and possesses strong interpersonal and communication skills, a willingness for collaboration within a multidisciplinary team, and clinical proficiency.
  • Performs other duties as assigned

Qualifications

Required:

  • High School diploma or equivalent
  • American Heart Association BLS certification
  • ACLS certification
  • Active Illinois Department of Public Health Paramedic License 
  • System entry required for Southern Fox Valley EMS system and remain in good standing
  • At point of hire and annually, required to have a valid driver’s license and an annual driving abstract
  • Physical Demands Requirement:
    • Physical demands required for this role include lift/carry up to 50 pounds and a push/pull force up to 50 pounds

Preferred:

  • Solid written documentation/communication skills
  • PALS certification
  • Applicants are encouraged to acquire community paramedic certification within 24 months of employment
