Description
The Care Coordinator NMPP reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
Responsibilities:
- Addresses the care management needs for high and moderate health risk patients as well as assistance with low risk or new patients.
- Review patient records and/or claims analysis to identify care management and care coordination services.
- Collaborate with the primary care physician to ensure smooth handoffs from Care Managers and primary care physicians (PCPs).
- Communicate with and disseminate information to the patient and other care team members to ensure consistent, seamless, and coordinated care. Actively engage as an integral, collaborative member of the patient's care team.
- Assist in facilitating care coordination for the patient across the care continuum, ensuring that the patient is receiving the highest level of quality care in each setting.
- Document required clinical, social, financial, or other information using appropriate systems.
- Participate in continuous quality, performance, and outcome assessment and improvement initiatives to ensure the improvement of care management/care coordination and available clinical and social support services.
- Assist in the development of care management tools needed to support the patient population.
Qualifications
Required:
- Minimum 5 years nursing experience.
- Relevant clinical experience and current knowledge of healthcare trends related to emerging models of population health management.
- Knowledge of chronic conditions, evidence-based medicine, care coordination, care management, and psychosocial and behavioral factors affecting health.
- Familiarity with patient-centered care models and techniques.
- Ability to prioritize and manage multiple high-risk, complex patients.
- Graduate of accredited 2 or 3 year registered nursing program.
- BSN within 2 years of hire
- Completes 12 continuing education units yearly.
- Ability to work with multiple members of a care team and maintain positive working relationships.
- Demonstrated ability to solve problems in a fast-paced environment.
- Experience with participating in clinical programs, such as disease management, health coaching, or preventive health programs.
- Strong problem-solving and decision-making skills.
- Demonstrated leadership experience.
- Strong commitment to improve the healthcare delivery system, with an emphasis on providing the highest-quality patient care.
- Case Management certification preferred, required within 12 months of hire.

- Bachelors of Science in Nursing
