Description

Scheduling Needs: 10:00am to 7:30pm Monday through Thursday and 10:00am-2:00pm on Fridays.  Occasional weekend hours and other hours as required due to patient needs.

Prepare and deliver weekly behavioral groups to coach patients on making lifestyle health changes using an empirical, data-driven approach. Different types of patient groups include: The Phase 1 (active weight-loss classes), The Phase 2 (Maintenance) classes for patients who have completed Phase 1, Quickstart Classes for patients just entering the HMR Weight Loss Program at Centegra Health System, Information Sessions, other special groups as needed. HMR at Home Quick Start calls. Maintains timely record keeping of individual and group summary data on all active patients. Follows up with current patients via telephone, email and periodic individual meetings to check in on program progress and compliance. Promotes quality patient care. Communicates regularly with the staff to promote a more consistent, integrated and effective program. Collaboration with multi-disciplinary team. Attend national HMR trainings to stay current with the latest treatment methodologies. Administrative Responsibilities: Attend meetings to discuss administrative and patient care issues. Compile a patient agenda for discussion at weekly patient care meetings. Coordinate and compile clinic behavioral data (monthly & annually). Be availability for marketing and promotion of medical weight loss to internal and external stakeholders. Participate in marketing efforts. Share responsibility with all staff to answer phones, respond to email and digital inquiries and other clinic duties. Adhere to and is responsible for the safety standards of CHS including the completion of the mandatory organizational safety standards on an annual basis. Maintain confidentiality for our patients and their families as outlined in the CHS Patient Confidentiality Policy and shows the same level of respect for every Associate with CHS. Qualifications

Required: High degree of professional demeanor, integrity and confidentiality. Excellent oral, written and interpersonal skills. Ability to function in an independent role and exercise sound judgment. CurrentAHA BLS Providercertification required. High School Diploma or Associates degree in health-related science or education. Evenings required and some weekends may be required. Demonstrated ability to communicate, problem solve, and work effectively with people using good judgment. Preferred: Background in motivational interviewing, active listening skills, mindfulness based and other evidence based coaching techniques preferred. Bachelor degree in health-related science or education. 2+ years of prior healthcare experience. Experience working in medical weight loss clinics.
