The Ambulatory Care Coordinator Managerreflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.


  • Provides leadership in developing a high-performing team that incorporates leadership principles and vision in performing case management functions.
  • Develop, implement, and maintain effective orientation, training, and competency programs for team.
  • Develop programs and work flow processes to address complex and chronic disease states for vulnerable health populations including adult, pediatric and behavioral health patients with multiple co-morbidities.
  • Evaluate and provide guidance and support to overseeing productivity, reporting, and quality metrics.
  • Provides leadership in developing a high-performing team that incorporates leadership principles and vision in performing case management functions.
  • Facilitate effective communication with team and resolve problem/issues timely.
  • Conduct staff meetings to communicate pertinent information and encourage staff participation in problem resolution activities.
  • Establishes key performance indicators utilizing industry or historical benchmarks and develops and implements a continuous quality improvement plan designed to improve performance and efficiency.
  • Responsible for ensuring effectiveness of all programs to optimize care across the continuum related to quality and cost.
  • Assumes responsibility for professional development by participating in workshops, conferences, and/or in-services.
  • Keeps abreast of trends in healthcare and incorporates them into practice standards and department initiatives to improve service delivery.
  • May perform other assignments and duties, as area needs require.




  • Bachelor's degree in health care field.
  • Minimum 5 years of case management/care coordination experience.
  • Outstanding leadership, organizational and planning skills with excellent organization, communication, team-building, and time management skills.
  • Commitment to a high standard of patient service and support of continuous quality improvement.
  • Functions as a leader, team member and fosters collaboration between all parties.
  • Strong interpersonal communication skills - verbal and written.
  • Exhibits creative and positive problem-solving abilities.
  • Thinks conceptually and applies concepts in a practical application.
  • Plans, organizes and effectively presents ideas and concepts to individuals and groups.
  • Working knowledge and experience with PC-based applications, i.e. Epic, Excel, PowerPoint, Outlook.
  • Valid driver's license and reliable vehicle for travel.


  • Master's degree.
  • Professional Certification in area of expertise.
  • Experience managing teams.
